Collecting and sharing sensitive information - collaborative guidance document

Over on the private Musical Inclusion Facebook page (dm me to join if you're part of a Youth Music -funded Musical Inclusion programme), one member asked if anyone had found a good way to ask people to describe their challenges, without feeling too invasive. How do you do it? Do you know how other professionals handle this? 

The conversations

The topic prompted a flurry of responses on Facebook, and continued through comments on posts on the Network, emails, a collaborative 'Google Doc' and culminated in a breakout discussion at Gathering 3 of the Musical Inclusion -funded projects in June 2014.
