Monitoring software suggestions

We're running a Youth Music funded project at our residential centre for NEET young people this year and want to keep our monitoring in one place. I'm after some suggestions of some software we can download onto a tablet to keep track of our musical and personal outcomes. We need it to be stored on the tablet and downloaded to a computer regularly as we don't have Wifi on our site.
For our musical outcome we need to keep track of participants self assessment and tutors assessment of musical skills and improvement using Youth Music's musical development scale at different points in the programme and feedback after each session.
For our personal outcomes we need to keep track of participants' confidence and emotional stability both within the music project and in relation to the wider programme.
Indicators for outcome 2
1. Young people will report an improved level of confidence and emotional stability. 2. Music leaders will report an improvement in participants confidence with music making and emotional stability within sessions and performances. 3. Amber staff will report an improvement in participants confidence and emotional stability when engaging with the wider programme.
Sources of evidence
For Indicator 1
Use Youth Music Well-being Scale to record the impact the sessions are having on participants mental state. Feedback will be completed on a tablet to ensure outcomes are all in one place.
